Who Qualifies

• You received a written notice from Oglethorpe Inc. stating that your personal information may have been impacted by the June 2025 data breach

• You are able to submit a valid claim form by the deadline of July 8, 2026

What You May Receive

If you are an eligible class member, you may receive one of the following: (1) up to $2,500 in reimbursement for documented out-of-pocket losses — such as unreimbursed bank fees, credit monitoring costs, or other expenses reasonably traceable to the alleged data breach — provided you submit supporting documentation; or (2) if you do not have documented losses, you may receive an estimated one-time cash payment of $75, which is subject to pro rata reduction depending on the total number of valid claims submitted. Additionally, all eligible class members may receive one year of free medical data monitoring services, regardless of which payment tier applies to them. Actual payout amounts are not guaranteed and may vary.

How to File a Claim

2. Complete the claim form in full, providing accurate personal information to verify your eligibility as someone who received a data breach notice from Oglethorpe Inc.
3. Gather and attach any required supporting documentation. If you are claiming reimbursement for documented losses (up to $2,500), you must include evidence such as bank statements, receipts, or other records showing losses reasonably connected to the alleged breach. No documentation is required if you are claiming only the standard cash payment.
